About your prescription medication: Written information about this medication has been provided for you. Please read this information before you take this medication. If you have questions concerning this medication, a pharmacist is available during normal business hours to answer these questions.

FDA Warning

  • This medication has been shown to cause thyroid cancer in some animals. It is not known if this occurs in humans. If thyroid cancer occurs, it may be deadly if not found and treated early. Call your doctor right away if you notice a lump or neck mass, have trouble breathing or swallowing, or have hoarseness that will not go away.
  • Do not use this medication if you have a health condition called Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), or if you or a family member have had medullary thyroid cancer

Important Information: 

This should only be taken by the patient whose name appears on the prescription label.


Do not use semaglutide if you are pregnant, planning to become pregnant, or currently breastfeeding.

  • Active Ingredients
    • Semaglutide is a glucagon-like peptide-1 (GLP-1) receptor agonist used to improve glycemic control in adults with type 2 diabetes and to aid in weight loss in certain adults.
      • Semaglutide’s most common side effects are constipation, diarrhea, nausea, vomiting, fatigue, headache, upset stomach and/or stomach pain. Swelling or redness at the injection site may also occur.
      • Some patients may also experience pancreas or gallbladder problems while using semaglutide. Stop using semaglutide and seek emergent medical care if you have severe abdominal pain, with or without vomiting, that will not subside.
  • Precautions and Contraindications 
    • Do not take this medication if you are allergic to semaglutide or any of the inactive ingredients.
    • Stop taking this medication and seek emergency medical care if you have signs of an allergic reaction like rash, hives, itching, red, swollen, blistered, or peeling skin with or without fever, wheezing, tightness in the chest or throat, trouble breathing, swallowing, or talking, unusual hoarseness, or swelling of the mouth, face, lips, tongue, or throat.
    • Do not take if you are pregnant, may be pregnant, or breastfeeding.
    • Do not use this medication if you have a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2 (MEN 2).
    • Notify all of your healthcare providers about any new medications medications you’ve been prescribed. 
    • Notify your provider if you have ever had pancreatitis. Seek medical care immediately if you have severe stomach pain, severe back pain, severe upset stomach or you begin vomiting. 
    • Notify your provider of any signs or symptoms of gallbladder problems like pain in the upper right belly area, right shoulder area, or between the shoulder blades; change in stools; dark urine or yellow skin or eyes; or fever with chills.
    • Notify your provider of any changes in your eyesight or vision.
    • Low blood sugar may occur  while taking this medication. This  may be increased when this medication is used with other medications for diabetes. Some signs and symptoms of low blood sugar include headache, dizziness, confusion, fast heartbeat, shaking, feeling tired or weak, hunger, or sweating. Call your doctor right away if you have any of these signs. Taking liquid glucose, or fruit juices can help relieve these symptoms.  If you are taking additional medications for diabetes, notify your provider as your dose may need to be adjusted.
  • Directions for Use
    • Inject subcutaneously once weekly on the same day each week, at any time of the day, with or without meals. You may inject into the abdomen, front of your thigh, or the back of your arm. Rotate the site each week. Below is a diagram showing common injection sites.
    • If you need to change your injection day, make sure there are at least 48 hours between doses.
    • Do not mix with any other injectable medications or products intended for weight loss.
  • For detailed instructions on how to inject Ozempic® and Wegovy® pens, please refer to the instructions for use enclosed in the box with your medication.
  • For Ozempic®, you will need to insert a new needle for each injection.
  • For Wegovy®, your single-dose pen already has a needle inside. Remove the cap only before injecting.
  • Use as directed by your prescribing provider, as they may tailor your dosing to your individual response.
  • After injecting, dispose of your used pen or needle in a sharps disposal container. If you do not have one, you can use a plastic bin such as an empty milk jug or laundry detergent container.
  • Needles are for a single use only and should never be reused or shared with others.
  • Storage
    • Store your Ozempic® pens in the refrigerator between 36°F to 46°F (2°C to 8°C) in the packaging provided by the pharmacy until you need it. After the first use, it may be stored out of the refrigerator, up to 86°F (30°C) for 56 days.
    • Store your Wegovy® single-dose pens in the refrigerator between 36°F to 46°F (2°C to 8°C) in the packaging provided by the pharmacy until you need it. If necessary, prior to use, it may be stored out of the refrigerator, up to 86°F (30°C) for 28 days.
    • Protect from freezing and light.
    • Keep out of reach of children.
    • Under certain circumstances a prescription drug’s effectiveness may be affected by exposure to extremes of heat, cold, humidity, or light exposure.
  • Missed Dose
    • If you miss a dose of the medication, take it as soon as you remember within 5 days. If more than 5 days have passed, skip the missed dose entirely and resume your medication at the next scheduled weekly dose. Do not take extra medicine to make up for the missed dose. Check your prescription label for the correct dose prescribed by your medical provider.
